What did we learn from the 2010 California whooping cough epidemic?

Because whooping cough (pertussis) is almost as contagious as measles (affecting ~12-17 individuals with each case), clinicians are required to report cases of this bacterial respiratory tract infection to the state's department of public health. In 2010, California had the highest number of cases of whooping cough in 60 years. A new study scheduled for publication in The Journal of Pediatrics describes the 2010 whooping cough epidemic and details strategies to decrease the incidence of this infection.

Kathleen Winter, MPH, and colleagues from the California Department of Public Health (CDPH) evaluated 9,154 cases of whooping cough with onset between January 1 and December 31, 2010; 809 cases were hospitalized and 10 resulted in death. All deaths and most of the hospitalized cases (62%) were in infants less than 3 months of age, and infants less than 6 months of age had the highest disease rates. In the population aged less than 6 months, Hispanics had the highest incidence of whooping cough. However, in children and adolescents 1-18 years of age, Whites had the highest incidence.

It is recommended that infants should receive 4 doses of DTaP (diphtheria, tetanus, and pertussis) vaccine by 18 months of age, and children should receive whooping cough "booster" doses at 4-6 years of age (DTaP) and 11-18 years of age (Tdap). Adults are also encouraged to receive the Tdap booster because immunity from both the disease and the vaccine wanes over time. The number of cases of whooping cough was elevated in pre-adolescents, even when they are fully vaccinated, indicating that protection from the 5-dose DTaP series may wane before the Tdap booster is given. However, the authors believe that the decrease in cases of 11-14 year olds suggests that Tdap is effective for adolescents.

In response to the sharp increase of cases in 2010, CDPH implemented a public health campaign to distribute educational materials to health care providers and the public to stress the importance of rapid diagnosis and treatment, especially in young infants, recommend vaccination for adults older than 64 years of age, under-immunized children 7-9 years old, and pregnant women, and provide free Tdap booster vaccines to hospitals, community health centers, and Native American health centers for pregnant and postpartum women and other infant contacts. To decrease the occurrence of whooping cough in infants who are too young to be vaccinated, it is important to immunize household and family members who will be in close contact with the baby (a strategy known as "cocooning") and increase the immunity of the population as a whole to decrease infants' exposure to pertussis.

Prior to the epidemic in 2010, only 23% of California birth hospitals had policies to offer Tdap to postpartum women. In 2011, the Centers for Disease Control and Prevention recognized the extreme vulnerability of young infants and recommended universal Tdap immunization for pregnant women (after the 20th week of gestation) who previously had not received Tdap. According to Ms. Winter, "In the absence of better vaccines, it is imperative that strategies to protect young infants directly, such as maternal vaccination during pregnancy, be evaluated for effectiveness. In addition, it is critical that providers continue to be vigilant and promptly diagnose and treat young infants with whooping cough."

Scalia says no 'falling out' with Roberts

(AP) ? Justice Antonin Scalia says he hasn't had a "falling out" with Chief Justice John Roberts over the Supreme Court's landmark 5-4 decision validating much of President Barack Obama's health care overhaul.

In an interview to be aired Wednesday on CNN's "Piers Morgan Tonight," the justice said despite reports that he and Roberts had clashed, there is not a personal feud going on between the court's two leading conservatives.

"There are clashes on legal questions but not personally," Scalia said of the court.

The Supreme Court earlier this month upheld much of Obama's signature health care law, with Roberts siding with the court's liberals to uphold the hotly debated core requirement that nearly every American have health insurance. The decision allowed the law to go forward with its aim of covering more than 30 million uninsured Americans.

Since then, Roberts has been the focus of derision from some of the nation's leading conservatives, and there have been reports of fractures in the relationships on the court's conservative wing, of which Roberts and Scalia are members.

"No, I haven't had a falling out with Justice Roberts," Scalia said, when asked about a purported clash between him and Roberts.

"Loud words exchanged, slamming of doors?" prompted Morgan.

"No, no, nothing like that," Scalia said.

Scalia also defended the court's 2-year-old decision in Citizens United to give corporate and labor union interests the right to spend freely to advocate for or against candidates for state and local offices.

"I think Thomas Jefferson would have said the more speech, the better," said Scalia, when asked about so-called super PAC spending on national elections. "That's what the First Amendment is all about. So long as the people know where the speech is coming from."

Scalia also said in the interview that the case that brings about the "most waves of disagreement" is still the decision that decided the 2000 presidential election between George W. Bush and Al Gore. But the justice said his normal answer to people who ask about Bush v. Gore is "get over it."

Scalia said it was Gore who decided to bring the courts into the battle. "The only question in Bush v. Gore was whether the presidency would be decided by the Florida Supreme Court or the United States Supreme Court," Scalia said. "It was the only question and it's not a hard one."

Scalia said he had no regrets about the court's decision.

"No regrets at all," the justice said. "Especially because it's clear that the thing would have ended up the same way anyway. The press did extensive research into what would have happened if (what) Al Gore wanted done had been done, county by county, and he would have lost anyway."

Scalia is beginning a book tour touting his new book, "Reading Law: The Interpretation of Legal Texts" with co-author Bryan A. Garner.

British PM: Afghan and Pakistan in joint fight

KABUL, Afghanistan (AP) ? British Prime Minister David Cameron called on Afghanistan and Pakistan on Thursday to make common cause against insurgents in the two countries' border zone. He spoke in Kabul in a visit aimed at adding British weight to American diplomatic attempts to get Pakistan to become more cooperative in ending the Afghan war.

Cross-border fighting and stalled negotiations with the Taliban were the main topics of discussion at a three-way meeting between Pakistani Prime Minister Raja Pervaiz Ashraf, Afghan President Hamid Karzai and Cameron.

The British, U.S. and Afghan governments have long criticized Pakistan for not doing enough to stop cross-border attacks. Many insurgents groups are based in safe havens in Pakistan's lawless tribal areas along the Afghan border. Islamabad also charges that insurgents cross the border from the Afghan side and attack Pakistani security forces.

"The terrorists that are trying to wreck Afghanistan are by and large the same terrorists that are trying to wreck Pakistan," Cameron said. "We should be together in one single fight."

Britain's involvement could help America's effort to push Pakistan to crack down on the safe havens and also to use its connections with some Taliban leaders to promote peace talks. Pakistan has longstanding historical ties with Britain but its relationship with Washington has been rocky at best, with their lowest point coming late last year after American airstrikes killed 24 Pakistani soldiers near the border.

Britain also sees the border areas as a main base for militants planning attacks against the West, although Cameron said that less than half of terrorist plots against Britain now originate from Pakistan's border regions ? down from two-thirds as recently as 2010. Three men were charged Wednesday in London with travelling to Pakistan for terrorist training.

"We are making progress, and this is our fight as much as your fight and the Pakistanis' fight," Cameron said.

A joint statement issued after the three leaders met said they would work together to eliminate terrorism and agreed that peace in Afghanistan would help Pakistan's security. Pakistan is facing its own Islamic insurgency and has been fighting militant groups, mostly in its tribal areas.

Pakistan's Ashraf, who took office last month, and Karzai also held separate talks together.

In the statement, Karzai stressed the need for a political solution to end a war that has lasted more than 10 years, and said talks should include the Taliban and other insurgent groups. It added that the talks should be led by Afghanistan, facilitated by Pakistan and supported by Britain and other countries.

The Taliban has refused peace talks with Karzai, calling him a puppet of the United States. Instead, they have held talks directly with American officials. They broke off talks earlier this year, saying the U.S. reneged on a promise to release Afghan prisoners from Guantanamo Bay. The talks have so far shown no signs of restarting.

Pakistan is seen as key to breaking this impasse because of its historical ties with the Taliban.

"The peace process is the most important of our pursuits for Afghanistan. It remains the highest priority," Karzai said.

Karzai said having talks among the leaders of Pakistan, Afghanistan and Britain was Cameron's idea and that there had been minister-level meetings among representatives of the three countries.

"Today is the first time that we are sitting at the high-level of the leaders," Karzai said. "The purpose of the meeting is to find an effective way to battle against terrorism and what role Pakistan can play now in the security situation in the region and the peace process."

Cameron had for some time hoped to bring together Pakistan and Afghanistan's leaders in an effort to boost trust between the two sides. Officials hope Thursday's talks will lead to more regular meetings between Karzai, Ashraf and their militaries to work on jointly tackling insurgents.

The Kabul meeting marked the end of Cameron's two-day visit to Afghanistan, intended to guide decisions on how fast Britain will withdraw its 9,500 troops before international forces leave by the end of 2014.

In separate talks with Karzai, Cameron discussed the drawdown. He also planned to stress the need for "credible, inclusive and nationwide" presidential elections in 2014, his spokeswoman said, on customary condition of anonymity in line with policy.

Cameron also met in Kabul with the top U.S. commander in Afghanistan, Gen. John Allen, who gave him a positive message on the country's overall security picture as they discussed recent violence in northeastern Afghanistan.

Allen said the process of handing security duties to local forces was progressing well, though the Afghan army still needs to improve its command and control capabilities, Cameron's spokeswoman said.

Cameron rejected claims Afghanistan could be mired in violence once foreign troops withdraw, insisting local forces are prepared to handle insurgents.

Taliban fighters should "be in no doubt that there is a very clear and strong plan to transfer lead security responsibility from very capable (NATO-led) ISAF forces, including British forces, to very capable Afghan forces," he said.

"I am content that they will be able to defeat an insurgency and that Afghanistan will have a secure future," Cameron added. "Of course we would make even further progress if there were to be successful political talks and if the insurgency was to come to an end on that way."

Although fighting has not abated in the south, it has escalated in the east where coalition forces have focused their efforts against insurgents.

NATO said two of its service members were killed by roadside bombs in eastern Afghanistan. It provided no other details, but the killings brought the death toll among foreign troops to 27 so far this month and 242 this year. In other violence on Thursday, a roadside bomb in northwestern Faryab province killed eight civilians and wounded six others, a spokesman for the governor said. They were in a mini-bus headed on a shopping trip one day ahead of the start of the holy month of Ramadan.

NATO also denied that it had contracted 24 fuel tankers destroyed on Wednesday by a bomb in northern Samangan province. They said the trucks belonged to civilians and had nothing to do with NATO. The Taliban claimed responsibility for destroying the vehicles.

Cameron also discussed the issue of "green-on-blue" killings on Wednesday in Helmand, southern Afghanistan, where British forces are mainly based. The term refers to the targeting of NATO troops by Afghan members of the security forces. There has been a rash of such killings in the last years.

He and Karzai also signed an agreement for 90 British military officials to staff an Afghan army officer training academy to be opened near Kabul. A total of 30 Danish, Australian and New Zealand soldiers also will staff the center, due to open before international forces leave at the end of 2014.

"I believe this is an important step for the Afghan people, this means having a stronger Afghan army, which is better trained, equipped and skilled," Karzai said.

The UX31A-R5102F is equipped with a third-generation 1.7GHz Intel Core i5-3317u processor and 4GB of RAM. It's the same processor seen in the Sony VAIO T13 (SVT13112FXS), but the Zenbook left the Sony behind in PCMark 7 thanks to its speedy 128GB SSD, scoring 4,315 points as opposed to the Sony's 3,383. In Cinebench R11.5, which more directly measures processing speed, the Zenbook scored 2.41 points?a step ahead of the Sony (2.32 points) and well ahead of the Sandy Bridge-equipped ultrabooks seen earlier this year, like the HP Folio 13-1020us ($899.99 list, 4 stars) (1.93) and the Dell XPS 13 ($999.99 direct, 4 stars) (1.82). This performance also helped the Zenbook lead the pack in Handbrake and Photoshopmultimedia tests, which it finished in 1 minute 59 seconds and 4:33, respectively.

Though the third-generation processor boasts Intel's improved integrated graphics?HD Graphics 4000?it still struggled in our gaming tests. The Asus Zenbook UX31A was unable to run Crysis, and Lost Planet 2 ran at unplayable levels, producing 15 frames per second (fps) at medium quality and 1,024-by-768-resolution, and 6 fps when set to 19,20-by-1,080 resolution and higher detail settings. Though it's not the laptop to take to a LAN party, the UX31A-R5102F still performed well in 3DMark 06, scoring 5,290 points at medium settings and 2,133 at higher resolution and detail settings, more than adequate for all but the most strenuous graphics processing.

With a 50Wh battery sealed inside its aluminum chassis, the Asus Zenbook UX31A lasted 6 hours 21 minutes in MobileMark 2007. While this is a slight improvement over the previous iteration (6:09), and better than the Dell XPS 13 (5:58), it's still toward the bottom of the pile for battery life. The Sony VAIO T13 (7:18) and Toshiba Portege Z835-P370 ($1,049.99 list, 4 stars) (7:21) both offer an hour more, while the HP Folio 13-1020us (8:47) stretches that to nearly two and a half.

A lot has happened since the first Zenbook came on the scene. Asus has updated the gorgeous design with a high-resolution display, more comfortable keyboard with backlight, and new hardware, making an impressive machine all the better. But the competition hasn't been sitting still, and there are several models that offer better battery life, better port selection, and larger storage capacity. The best of these competitors actually comes from within the Zenbook family. The Editors' Choice for midrange ultrabooks is still the HP Folio 13 1020us. San Bernardino Bankruptcy: City Declares Fiscal Emergency

SAN BERNARDINO, Calif. -- San Bernardino declared a fiscal emergency Wednesday night, allowing the city to avoid a lengthy mediation process and head straight to federal bankruptcy court.

The declaration comes after the city announced last week that it would seek Chapter 9 protection, making it the third California city in recent weeks to make the rare move.

The City Council voted 5-2 to declare the emergency and file for bankruptcy protection amid a dire cash crunch that has officials worried San Bernardino can't meet payroll in August.

The vote was followed by another authorizing the city attorney to file for bankruptcy, but it was not clear when the planned filing would come.

Councilmen Chas Kelley and John Valdivia dissented on both votes.

Councilman Fred Shorett, who voted against bankruptcy last week, reversed his position Wednesday night and approved both moves.

"The horse is out of the barn ? the whole world knows we're insolvent," Shorett said, according to the San Bernardino Sun. "I will be supporting going forward with Chapter 9 and fiscal emergency."

The vote could make the city of 210,000 people the third in California to seek bankruptcy protection since last month, following Stockton and Mammoth Lakes.

The city is facing a $45.8 million budget shortfall this year.

Last week's announcement of the bankruptcy plan has further stressed San Bernardino's finances by prompting a dozen employees to put in for retirement with hopes of cashing out accrued vacation and sick time, and it has spurred vendors to demand cash instead of credit, said Gwendolyn Waters, a spokeswoman for the city manager's office.

The debate over bankruptcy in San Bernardino has also raised questions about the city's financial management. Last week, City Attorney James Penman told the public that 13 of the last 16 budgets presented to the city council had been falsified, masking the city's deficit. The finance director, who is new to the job, said officials had borrowed cash from restricted funds to cover payments, and eventually ran out of money to pay the funds back.

Officials say the housing crisis ? which walloped property and sales tax revenues ? and the loss of state redevelopment funds took a toll on the city's budget. San Bernardino is located about 60 miles east of Los Angeles.

LOS ANGELES (AP) ? Amid ongoing legal issues, a recent car crash and run-in with paramedics, Lindsay Lohan has secured her next movie role.

Producer Braxton Pope says Lohan will star in "The Canyons," a film by "Less Than Zero" author Bret Easton Ellis about sex and ambition among 20-somethings in Los Angeles. Paul Schrader, who wrote "Taxi Driver" and directed "American Gigolo," is set to direct the film.

Lohan's publicist, Steve Honig, confirmed Wednesday that the 26-year-old actress had been cast in "The Canyons" and added that she has been on set since last week.

"A big part of the reason she did this was because of Paul Schrader and the opportunity to work with such an iconic filmmaker," Honig said.

Lohan recently completed work on the Lifetime movie "Liz & Dick," in which she played Elizabeth Taylor. During that production, she crashed her Porsche into a dump truck and a week later was treated by paramedics for exhaustion and dehydration. Police haven't released their findings in the June 8 car crash, and no charges have been filed.

Last week, Lohan's attorneys confirmed that she has resolved a case filed by three men she took on a high-speed ride that resulted in her second driving under the influence arrest in 2007.

Lohan remains on informal probation for taking a necklace last year from a jewelry store without permission. She is not required to check in with a judge or probation officer, but could face a jail term if she is re-arrested.

In "The Canyons," Lohan plays opposite James Deen, who is best known as an adult film star with thousands of movies to his credit. Gus Van Sant also has been cast in the film, Pope said.

Schrader describes the characters as "beautiful people doing bad things in nice rooms."

"Lindsay Lohan and James Deen not only have the acting talent," he said, "they also have that screen quality that keeps you watching their every move."

Filming is set to continue next week.
